SUBMIT PAYMENT: LOCATE MY ACCOUNT DETAILS

Alright, so I need to finalize my invoice with Dr. Smith. I went to their website and it's asking for a account ID number. Problem is, I have absolutely no idea where that resides. I've looked my emails, insurance paperwork, but nothing! Could someone please guide me? Is there a phone number I can reach out to? Perhaps written down somewhere?
